PSWG draft 7

Here it is. It's not perfect, but hopefully a little more cohesive. I've
included all minority positions. I won't be accepting any edits to minority
positions unless it's spelling etc. I've zipped up the appendices, so please
get back to me if that proves to be a problem.

On C12.19, I listed comments in order of receipt.  Please get back to me if
something is incorrect, like it should be "a", but the report says "b" Or if
you see any typos, etc, etc. 

I'd like to include a table in the executive summary showing the extent of
agreement we have reached. I think it is quite an achievement for such a
diverse group of stakeholders. I'd also be interested in feedback on that.

I still need to put together the security appendix, which I will send out
later today. (Ed did you do anything on that?)
