RE: Standardized Third-Party Authorization Form

Hey Guys/Gals,

The Third-Party Authorization Form looks great except for the following.
Please consider the following changes...

In Category 1, change first choice to read...
a) Request and receive 12 consecutive months of billing records, billing
history (including invoice and payment dates) and all meter usage data
including but not limited to CMEP and/or EDI 867 data posted on the MDMA
server used for bill calculation for the specific account(s), as specified
herein, regarding utility services furnished by the Utility in no less that
7 days in an industry standard normalized electronic format.


In Category 1, second section change third choice to read...
Authorization is given for the period commencing with the date of execution
for ______ (Limited in duration to three four years from the date of
execution.  If more than 12 months has elapsed since the authorization was
originally given, the Utility may NOT require either a copy of the original
form or a newly executed form before any subsequent action may be taken on
your behalf.)

I welcome discussion on these changes. I appreciate the opportunity to
provide input on this document.
